Died on December 20, 2011 in Mesa, Arizona. He is survived by his wife Florence, nee Dait; loving father of Cyndee (Bob) Miller, Kathy (the late Larry) Arendarczyk, Dr. Eugene (Carolyn), the late Michael and Paul (Sacha Blanchet); fond grandfather of Jenna, Paul Jason, Justine, Ric, and CJ; dear great-grandfather of Lana, Kylie, Ethan, Gavin, Skyler and Spencer.

He was a faithful servant of the Lord, loving father, dedicated physician, advisor, comedian, and a friend to many.

He received his Doctor of Medicine degree from University of Michigan in 1957 and retired from Family Practice after 40 years, leaving behind three-generation families in Chicago, IL. He held numerous positions in the Illinois Academy of Family Physicians and received the American Medical Association Physician Recognition Award.

He was proud to be an American. He served in United States Army during World War II, the Korean Conflict and Army Reserves MASH unit. Also, he was a prisoner of war in World War II by the Japanese in the Philippines. Received the Meritorious Service Award for being researcher and instructor in tropical medicine at Walter Reed Army Hospital.

He was a 32nd degree Mason and received the Meritorious Service Award for Medical Director by Ancient Accepted Scottish Rite, Valley of Chicago.

“Doc” was an active member at East Valley Adult Resources for 15 years; touching the Red Mountain Active Adult Center in exceptional ways. In 1999 when the Center transitioned from a modular facility to the Multigenerational Center he and his wife, Florence, played key roles in welcoming new members and enhancing the programs. As Chairman of the Advisory Committee and member of the Board of Directors, his leadership style of “participatory governance” built a strong base at the Red Mountain Center. He promoted intergenerational involvement in the billiards room, by teaching younger adults the protocol. He also played a key role in developing the ambassador program to welcome new members. His personality and positive drive was an inspiration to all those who had the privilege of working with him.

Funeral services will be held Tuesday, February 21, 2012 at 10:00 AM, Saguaro Hills Baptist Church, 1402 N. Power Rd, Mesa, AZ 85205, 480-981-1255. Military Service at 2:00 PM, National Memorial Cemetery of Arizona, 23029 N. Cave Creek Rd, Phoenix, AZ 85024, 480-513-3600.
