If you are already logged into your browser with
your ASU credentials, and if you have been
registered for our class (PHY121 line number 93009)
for more than 48 hours, then you should have no
trouble with the survey.  You may possibly be asked
to login to Google.  If asked, login with
your_ASUrite_ID@asu.edu, which will take you to
Google's SSO application for ASU.

If you have any issues, then detailed instructions
for accessing the survey can be found below.

WARNING:  In the survey form, above the words
"Your email will be recorded when you submit
this form," you MUST SEE your ASUriteID@asu.edu.
Else you are not signed in correctly.  Sign out
and then sign back in using your ASUrite ID.

You must be signed in to your browser with 
your ASU credentials in order to access the
form.  If you have trouble with this, you
can follow the instructions below:

1) Open a private browser window.  (On Firefox
for example, this is done by control-shift-P.)

2) In the private browser window, go to 
     Pre-Class Survey Form

3) You will land on a Google sign-in page,
which will say "Sign In to continue to Forms".
You need to sign in to your official, ASU-
affiliated, Google account.  The correct sign
in should be your_ASUrite_ID@asu.edu.

What if I don't know my ASUrite ID?

4) Now you should be taken to a standard ASU
Login page.  Login with your ASU credentials,
and complete the 2-factor authentication.

5) Now you should arrive at the survey form.
