Application forms for admission to the university are available here on the Web. You should begin by visiting the general web-site for ASU. Information on just about everything related to getting admitted can be found here. Electronic forms for admission to the university, in both pdf and Word formats are here. (Be advised that the Word file is very large and readable only with the most recent version of Microsoft Word.)
